Phase One: Intake and Initial Visit

Our care starts before treatment begins. Following initial referral or inquiry, our Patient Care Coordinator (PCC) verifies insurance, gathers prior authorizations, and contacts the patient to complete intake. This includes:

  • Medical history review
  • Surgical/injury background
  • Activity level assessment (occupational, recreational, athletic)
  • Goal identification
  • Alignment with referring provider’s clinical notes (when applicable)

 

Our licensed therapist then conducts the Initial Evaluation, including:

  • Functional movement screening
  • Special tests relevant to the condition
  • Subjective symptom analysis
  • Baseline outcome measurement tools (e.g., DASH, LEFS, Oswestry)

 

This phase closes with provider-patient discussion, expectation-setting, and plan of care mapping.

 

Phase Two: Customized Programming and Intervention

Each patient carries a unique history, timeline, recovery rate, and activity goal profile. At Limitless, treatments focus on:

  • Manual therapy, therapeutic exercise, neuromuscular re-education
  • Specialized services (vestibular, pelvic health, orthopedic, return-to-sport, etc.)
  • Functional mobility coaching with job- or sport-specific modification
  • Patient education strategies surrounding movement, posture, return-to-load
  • Behavioral reinforcement (home program compliance, mindset coaching)

 

Our therapists incorporate professional collaboration with coaches, athletic trainers, and physicians as needed.

  • Adjusting exercise complexity based on tolerance and benchmarks
  • Messaging goal status to referring providers or stakeholders when appropriate

 

Phase Three: Reassessment, Outcome Tracking, and Case Closure

Patient "graduation" is an earned outcome that rests on both objective improvements and subjective milestones. Final phases of therapy are structured around:

  • Final reassessment using validated outcome measures
  • Functional return-to-life testing (e.g., sport, work, independence)
  • Stabilization and maintenance strategy education
  • Reintroduction to fitness or performance settings (as appropriate)
  • Preventative accessory programming, if applicable

 

Upon discharge, referring providers receive summaries when requested, and patients are encouraged to schedule performance follow-up or re-check sessions as needed.

Therapists at Limitless are equipped to make post-discharge referrals and coordinate with outside resources (e.g., personal trainers, condition-specific coaches, or nutritionists).
